DepEd Grading Sheet: What the 2026 Rules Require

DepEd Grading Sheet: What the 2026 Rules Require

TL;DR: The grading sheet, formally the class record, is governed by DepEd Order No. 015, s. 2026, which repealed DO 8, s. 2015 on 4 June 2026. Most Grade 4 to 10 learning areas weight Written and Oral Works 20 per cent, Performance Tasks 50 per cent, Examinations 30 per cent; EPP/TLE and MAPEH use 20-60-20. Inside Examinations, ST1 30, ST2 30, Term Exam 40. An adjusted transmutation table applies for SY 2026-2027 only, where an Initial Grade of 70.00 becomes a Term Grade of 75, and from SY 2027-2028 grading is zero-based with no transmutation. DO 8 weights survive only for Grade 12 this school year. Supporting records must be retained for at least one school year.

Most of the grading sheet templates circulating in staffrooms and Facebook groups were built for an order that no longer exists. This guide sets out what the current order actually requires you to record, compute and keep.

What DepEd Means by a Grading Sheet

Three different documents get called a grading sheet, and they are not interchangeable.

DocumentWhat it isWho holds it
Class recordThe teacher's working record of all raw scores per component, per learnerSubject teacher
Learner's Progress Report (SF9)The reported grades communicated to parents or guardiansClass adviser
Learner's Permanent Academic Record (SF10)The permanent transcript of final gradesSchool records

DO 015, s. 2026 is direct about the class record's status: "Teachers shall maintain complete and accurate class records to support all grading decisions. These records shall be subject to validation by school heads and instructional supervisors to ensure transparency, consistency, and accountability." It is evidence, not a scratch pad.

DepEd Order No. 006, s. 2025 confirms its formal standing by listing the Class Record in Enclosure 1 as a standard school form for all teachers, sitting under Assessment of Learners Progress next to Learning Outcome Assessment, Test Result and Analysis, and the Report on Remedial and Intervention Program. SF9 and SF10 appear in Enclosure 2, tied to homeroom advisory duties rather than to every teacher. Our school forms guide maps the full SF series.

One exception worth knowing: for Kindergarten, DO 015 states that class records "shall be maintained primarily as qualitative documentation," and that numerical grades "shall not replace descriptive reporting for parents or guardians." Kindergarten to Grade 1 moved to descriptive grading in SY 2026-2027, Grade 2 follows in SY 2027-2028, and Grade 3 in SY 2028-2029.

The Weights You Enter

SHS Research Electives and Design and Innovation40%60%none
SHS Tech-Pro Electives15%65%20%
SHS Work Immersion20%80%none

Two structural points people miss. For Field Exposure, Arts Apprenticeship and Creative Production the Examinations component consists only of a Term Examination, with no summative tests, carrying the full 15 per cent. For Work Immersion, the 80 per cent Performance Task column is the industry evaluation from the workplace supervisor, and the 20 per cent is the learner's portfolio.

GMRC and Values Education carry an extra layer: the weights are split across cognitive, affective and behavioural domains, with the behavioural domain taking 30 per cent of the Performance Task allocation.

The Five Steps, in Order

  1. Total the raw scores for each component: Written/Oral Works, Performance Tasks, Examinations.
  2. Convert to a Percentage Score: PS = (raw score / highest possible score) × 100.
  3. Compute the Weighted Score: WS = PS × the component weight.
  4. Add the weighted scores to get the Initial Grade (IG).
  5. Apply the transmutation table, if it still applies to the grade level and school year.

Worked example, constructed for illustration. A Grade 6 Science class record, using the 20-50-30 weights.

ComponentPercentage ScoreWeightWeighted Score
Written/Oral Works74.0020%14.80
Performance Tasks86.0050%43.00
Examinations (ST1 78, ST2 82, TE 88, weighted 30-30-40 to 83.20)83.2030%24.96
Initial Grade82.76

An Initial Grade of 82.76 falls in the 81.80 to 82.97 band of the adjusted table, giving a Term Grade of 85. The figures above are invented to show the arithmetic; they are not taken from any real class record. The guide to computing DepEd grades works through more cases.

The Transmutation Table Applies This Year Only

For SY 2026-2027, Annex D of DO 015 prescribes an adjusted transmutation table for Key Stages 2 to 4, including Grades 2 and 3 that are still on numerical grading. Its defining feature: an Initial Grade of 70.00 is required to reach a transmuted Term Grade of 75.

Initial GradeTerm Grade
99.50 to 100.00100
90.06 to 91.2392
80.62 to 81.7984
75.90 to 77.0780
70.00 to 71.1775
65.34 to 69.9974
46.67 to 51.3370
0.00 to 4.6760

Initial Grades from 70.00 to 100.00 are distributed proportionally across Term Grades 75 to 100. Initial Grades below 70 map into 60 to 74, with 60 as the minimum reportable grade. The order describes this as a shift "from compensatory grading practices toward a system that emphasizes demonstrated achievement."

Beginning SY 2027-2028 the table goes away. Term Grades will be the computed Initial Grades directly, with 75 as the minimum passing grade, which DO 015 calls the zero-based grading system. Our transmutation table guide covers what changed against the old table, and the new grading system guide covers the wider reform.

The Grade 12 Exception

There is exactly one place where DO 8, s. 2015 still governs a grading sheet. DO 015 provides that "for SHS Grade 12, which has not yet implemented the Strengthened SHS Curriculum for SY 2026-2027, the weights in DO No. 8, s. 2015 shall apply together with the adjusted transmutation table."

So a Grade 12 adviser in a non-pilot school this year runs the old weights with the new transmutation table. That is a genuinely awkward hybrid, and it is the reason two teachers in the same building can both be right about different weights. Everything else about DO 8's status is covered in our DO 8, s. 2015 status guide.

From Term Grade to Final Grade

The Final Grade in each learning area is the average of the Term Grades, rounded to the nearest whole number. A Final Grade of 75 or higher indicates the learner met the minimum standards; below 75 triggers remedial intervention or academic support.

The General Average is the average of the Final Grades across all learning areas taken during the school year, also expressed as a whole number. For Key Stage 4 the General Average is computed using the equivalent units of each subject, with detail promised in a separate Strengthened SHS issuance.

Numerical grades must be accompanied by qualitative descriptors: 90 to 100 Achieving, 80 to 89 Benchmarking, 75 to 79 Connecting, 65 to 74 Developing, 0 to 64 Emerging.

What You Must Keep, and Where

DO 015 sets the record-keeping rules out plainly:

  • Retain assessment tools, rubrics, raw scores and feedback records for at least one school year, or longer if needed for audit, validation or appeals.
  • Store physical records in designated secure cabinets within school premises.
  • Store digital records in official DepEd systems, networks or authorised drives.
  • Access must be controlled, logged and documented, and records made available on request.
  • School heads conduct regular validation of grading practices.

Grades are submitted at the end of each term using official DepEd forms and digital systems, in accordance with DO 006, s. 2025. Many schools use an electronic class record for the arithmetic; whichever tool a school uses, the retention and validation duties above attach to the underlying evidence, not the spreadsheet. Our e-class record guide explains what that system is and who administers it.

Where Grading Sheets Go Wrong

  • Using a template built for DO 8, s. 2015. The weights, the transmutation table and the minimum reportable grade all changed.
  • Weighting the three examination pieces equally. It is 30-30-40, combined before the component weight is applied.
  • Adding an examination column to Work Immersion or Research electives. Those subject groups have no Examinations component.
  • Transmuting a Final Grade. Transmutation applies to the Initial Grade to produce a Term Grade. The Final Grade is an average of Term Grades.
  • Rounding at the wrong step. The Term Grade is written as a whole number; the Final Grade is the rounded average of Term Grades.
  • Keeping only the spreadsheet. The order requires the underlying tools, rubrics, raw scores and feedback records to be retained too.

Disclaimer: assessment and grading policy is set by DepEd issuance and is currently in a multi-year transition, with different rules applying to different grade levels in the same school year. Confirm the rules for your specific grade level and learning area with your school head before finalising any record.

Frequently Asked Questions

Which DepEd order governs the grading sheet now?

DepEd Order No. 015, s. 2026, the Revised Guidelines on Classroom Assessment, Grading System, and Awards and Recognition, issued 4 June 2026. It repeals DepEd Order No. 8, s. 2015 outright. Material that describes DO 8 weights and the old transmutation table as current is out of date, with one narrow exception for Grade 12.

What are the component weights in the class record?

For Grades 4 to 10 in English, Filipino, Mathematics, Science, Araling Panlipunan and GMRC or Values Education, the split is Written and Oral Works 20 per cent, Product and Performance Tasks 50 per cent, and Examinations 30 per cent. EPP or TLE and MAPEH use 20-60-20. Senior high school subject groups use their own table, ranging from 20-50-30 for core subjects to 20-80 with no examination component for Work Immersion.

How is the Examinations component broken down?

Within the Examinations component, Summative Test 1 accounts for 30 per cent, Summative Test 2 for 30 per cent, and the Term Examination for 40 per cent. Those three are combined first, and the result is then weighted by the component percentage for the learning area.

Is transmutation still used?

For SY 2026-2027 only, as a transition. An adjusted transmutation table applies to Key Stages 2 to 4, including Grades 2 and 3 still on numerical grading, where an Initial Grade of 70.00 corresponds to a transmuted grade of 75 and 60 is the minimum reportable grade. From SY 2027-2028 a zero-based system takes over, with no transmutation and 75 as the pass.

Do DO 8, s. 2015 weights still apply to anyone?

To one group. DO 015, s. 2026 provides that for SHS Grade 12, which has not yet implemented the Strengthened SHS Curriculum in SY 2026-2027, the weights in DO No. 8, s. 2015 shall apply together with the adjusted transmutation table. Everyone else is on the new weights.

How long must a teacher keep the supporting records?

At least one school year, or longer if required for audit, validation or appeals. DO 015, s. 2026 covers assessment tools, rubrics, raw scores and feedback records. Physical records go in designated secure cabinets on school premises and digital records in official DepEd systems, networks or authorised drives, with access controlled, logged and documented.

Is the class record an official school form?

Yes. DepEd Order No. 006, s. 2025 lists the Class Record in Enclosure 1 as a standard school form for all teachers, under Assessment of Learners Progress, alongside Learning Outcome Assessment, Test Result and Analysis, and the Report on Remedial and Intervention Program.

What is the difference between an Initial Grade and a Term Grade?

The Initial Grade is the raw computed grade, the sum of the weighted scores from all summative components before transmutation. The Term Grade is what gets reported, written as a whole number. In SY 2026-2027 the Term Grade is the transmuted Initial Grade; from SY 2027-2028 the two are the same figure.
